How to Apply
Each health insurer sets the premium for each plan, so there is no lower/lowest quote than here for any particular plan of individual or family affordable health insurance Colorado. There is no charge to apply through this website or for sending an application to us. We are paid by the health insurance company.

Choose One Application Method:
1.
Opens Pacificare's secure online application page. ALLOW POPUPS so the entry for county will work.
2.
If you are paying the first month by credit card, you can scan/email or fax the completed application form to us. If paying first month by check, mail the application to us with a check (made out to “Pacificare”) for the first period’s premium.
Provides automatic payment of premium from a bank account. Otherwise, mail a check each month
First month (only) can be paid by credit card.
3.
Request Application Form
Request by email, fax, or telephone and we will send the application to you.
4.
Apply by Telephone
Call (303) 722-2222 and we will complete the application for you over the telephone.

Effective dates are the first or fifteenth of the month, but never sooner than the day the application is received by Pacificare.
Underwriting may take about two to four weeks. For notice of approval to be made prior to the effective date, the requested effective date must give the health insurance company sufficient time to underwrite.
The application will not be accepted if the date of signature is more than 60 days prior to the effective date.
Do not cancel current health insurance until the new application is approved. If timing is a problem, a written cancellation conditional upon the new application being approved may provide flexibility. Verify with the current health insurance company that they will accept that.
